Complete the Westgate Permission Slip form linked here.

Submit payment of $340 for the trip either online through our payment portal or drop off cash or check at the front desk.
- Fill out, and send to your physician, any required medication/allergy forms that will help us keep your student safe (linked here and download using the left blue buttons). If your student already has medications and forms completed in the school health office, you don't need to take any further action.

Complete and return the Keystone Science School’s waiver (which will be sent home with the student once we begin school).

If your student is unable to attend, or you do not wish for them to participate this year, please still fill out the Westgate Permission Slip Form linked here! There will be an alternative opportunity offered at Westgate Community School for students who do not attend.
CHAPERONES: If you are interested in chaperoning, there is a spot on our permission slip to mark that. Please note, we will not have a final count of students to determine the number of chaperones needed until families complete registration and payment. You will hear from teachers on August 18th about whether you'd like to join us and begin the background check process!
PACKING LIST: We will cover all the required essentials needed for our trip once the number of students has been confirmed on August 18th. Thank you for your patience!
